The end portal is only supposed to be found as a block in lit end portals in strongholds and in the end "fountain" on the main island that appears after defeating the ender dragon, though through a few ways have been found to obtain this rare item.
Placing water on an End Portal in 1.2.13 results in it dropping as an item. This can be done by putting the water on the side of the bedrock of the exit portal.

Certain actions cause the exit portal in the end to regenerate. Mining a block and breaking it as the end portal regenerates over it makes the end portal item drop instead of the mined block.
