Nausea II is a stronger version of the Nausea I effect.
Obtaining
Eating Pufferfish
Nausea II would be given when eating a pufferfish from 13w36a (1.7 snapshot), when pufferfish were first added, to 1.16.2 Pre-release 1.[1] This is a temporary discontinued feature, as the effect is limited to 15 seconds.
