Slowness V was available through a strong slowness potion in versions 18w07a through 18w16a (1.13 snapshots). This was changed to slowness IV in 18w19a. Additionally, slowness VI is obtainable through strong turtle master potions. This is a temporary discontinued feature, as the effect only lasts for 10 seconds. This effect can also be applied to any other mob that accepts potion effects..

Voting for the Slowness V Effect
In the April Fools snapshot 23w13a_or_b you could vote to get infinite duration effects. These effects could be random, so you could get infinite Slowness V.
